Staff audit and photo permission Form
AUDIT PERMISSION - Every 12 months Inclusion Plus Family Support Inc. is required to undergo an audit of its management systems by an independent accredited auditing body as part of our Quality Assurance Accreditation - One of the steps the Auditors must take is to randomly inspect Staff member’s files to ensure that we adhere to the requirements of our Policy and Procedures. Accordingly, we need permission from staff to provide information from their files to the Auditors and are seeking your permission to do so. Please note that it is not mandatory for you to provide this permission - I understand that it is not mandatory for me to give this permission and I do so of my own free will - I hereby give my authority for my Personal file to be made available to the organisation’s selected External Auditor as part of the review of our Quality Assurance Systems.
PHOTO AND VIDEO IMAGE RELEASE STATEMENT - I hereby give my permission for Inclusion Plus to take, display and/or share photographs and/or video of myself in the course of my employment with them. These images may be used in all or some of the following ways - Media releases and media articles, including newspapers, radio, television, printed publications and other organisational marketing materials - Electronic publications and communications including Inclusion Plus Social Media Platforms and Website - I understand that publication of these images may result in me being able to be identified in connection with the images/videos published - I authorise the use of these images without compensation to me. All prints, digital files and digital reproductions shall remain the property of Inclusion Plus Family Support Inc - This permission will remain valid and effective until written advice has been provided to the organisation withdrawing this authority.
