- All participants must have adequate NDIS funding or be self-funded to access our programs.
- It is a requirement to have a signed Service Agreement with a Schedule of Supports in place prior to support provision, confirming NDIS funding allocation for this service.
- After the program has been provided, Inclusion Plus will invoice against the total support hours delivered, as well as invoice against any program/activity and transport fees.
- It is the responsibility of the participant (or their representative) to ensure that sufficient funding is available in their NDIS Plan to cover support fees, program/activity fees, and transport fees. If these fees are not claimable through your NDIS Plan, or if there is insufficient funding, then the participant/their representative will be responsible.
- Program/activity fees, transport fees, and other related fees may be applicable in addition to the figures on your Service Agreement.
- Transport fees are indicative and may be subject to change.
- Please ensure you are carefully reviewing and selecting activities appropriate and suitable for the attendee based on their age, medical conditions, diagnosed disabilities and ability level.
- Activities in the community are subject to weather conditions, and any centre-based support will incur additional fees.
- As detailed in your Service Agreement, to avoid incurring any undelivered support charges adequate notice must be provided when cancelling support - 7 days notice is required to refund activity fees.
- 3 days notice is required to avoid full support fees.
